Host Evaluation Form
If you have had a student ministry team from Elim serving at your church/ministry, please take a few moments at the end of the semester to fill out our Host Evaluation form below. This form will help us in our evaluation of each student's effort and participation as part of the team and will be used for grading purposes.  Your evaluation will remain confidential; the students will not be given access to the form.
 
This evaluation form should be completed by the person who directly interacted with the team. It is important for the student to receive objective, honest evaluation of his/her performance and attitude while serving your ministry, and your prayerful and conscientious evaluation of the student's performance while under your supervision is necessary. It is important that ALL sections of the evaluation form be completed. For maximum effectiveness, we recommend that by completion of the ministry, you discuss these observations with the student (not necessarily show him/her this form). Your comments will be especially valuable later on during the student's placement counseling.

 
 
 
 

Please use the following code for completing this form:
1 = Low performance level, serious reservations about the intern's performance.
2 = Weakness is evident, immaturity evident, imporvement slow.
3 = Average, satisfactory, showing areas of needed growth.
4 = Above average, highly responsible, criteria often met.
5 = Excellent, well above average, consistently fulfills criteria.
