NOTE: After a ministry internship has been completed, the student has until the end of March of the following academic year to submit all documentation to the Dean of Ministry without having to repay all tuition fees for that internship.

1. As the Internship supervisor—the minister who will actually oversee the student’s involvement—please review the “Supervisor’s Manual.

2. Please complete this form and submit it to the EBI Dean of Ministry for final approval before a commitment is made. Please complete all information requested in this form as completely and detailed as possible – even if you think we already have this information on hand.

3. IMPORTANT: If the supervisor is not the senior pastor/director of the ministry, is he or she fully informed and in agreement with this application?
