Internship Evaluation Form

Internship Evaluation

NOTE: In order for Elim Bible Institute to grant the student the academic units accomplished by the internship service, your prayerful and conscientious evaluation of the intern’s performance while under your supervision is necessary. It is important that ALL sections of the evaluation form be completed. For maximum effectiveness, we recommend that at least by completion of the internship you discuss these observations with the intern (not necessarily show him/her this form). Your written comments will be especially valuable later on during the student’s placement counseling. See further comments in the Supervisor’s Manual.
