Team Leader Weekly Report: to be filled out weekly by Student Ministry Team Leaders and submitted to the Ministry Department by 1:00 PM the day after your student ministry. Adjunct Student Ministry Report Form: To be filled out by those who have been approved by the Dean of Ministry to continue on in a ministry the student has already been working with for six or more months prior to enrolling at EBI. This report must be filled out weekly and submitted to the Ministry Department by 1:00 PM the day after the ministry takes place. Team Evaluation: To be filled out by your student ministry team during your last team meeting of the semester. Please give your honest input, as this will help in evaluating your team and evaluating the ministry to plan for the next semester. Personal Evaluation Form: To be filled out by every EBI student after your last student ministry each semester. You have the opportunity to evaluate yourself, your effort, and your participation as an individual on your student ministry team! Your grade for your student ministry will be partially determined by receipt of this form as well as your input on the form itself. |
